1.    Proof of a bona fide sale or transfer of the vehicle filed with the DMV

2.    Copy of the lease or rental agreement providing the rentee/leasee information above and the term of the agreement

3. A police report or insurance claim citing the date of the theft.


If all the information is not accurately completed and verified by the DMV, the affidavit will be disregarded and you will be held responsible for the violation.
